Our Services

Permanent Residential Care

After months or years of debating the best care solution for your loved one, you have decided it is time for a residential home—one that provides the quality 24/7 care, safety, support and positive stimulation that your loved one needs and deserves—at a price you can afford. Visit other facilities, then enjoy a tour of The Country Home and spend some time with our management team. If you join us, The Country Home will assure the highest level of independent functioning and quality of life for your relative. You will have worry-free peace-of-mind, knowing you made the best possible decision. You may stay as closely involved with your loved one as you like, visiting anytime. Bring any questions, concerns, suggestions to our administrator or other staff so we can address them immediately.

Services include:

  • Medication management by certified staff; weekly check-ups by RN; three nutritious, home-cooked meals and snacks; assistance with bathing, grooming, dressing, eating, walking, toileting, etc; personal laundry and linen service, beauty/barber salon on site; 24/7 supervision by dedicated, long-term staff with specialized Alzheimer's disease training; mentally and physically stimulating activities, social events, pet therapy, holiday parties, entertainment, spiritual services; individualized care plan and services to match each resident's changing needs; close coordination with family and health care professionals; support group and informational services for caregivers; referrals for any outside services needed, etc.
  • A team of our staff members—our registered nurse, the admissions manager, the administrator--will obtain all relevant information from the prospective resident, the family and health care providers so we can do a thorough assessment and initial care plan. This usually involves a home or hospital visit. Sometimes the assessment is done at The Country Home.
  • $4,800 per month for a private room; $4,300 per month for a shared room
  • The monthly fee is all-inclusive except for personal products, incontinence products, oxygen, medications, special dietary and other personal items. Fees do not increase as "care level" or number of medications increase!
  • Prior to admission, new residents must have a two-step TB test, a MRSA test and a flu vaccination, during flu season.
  • Medical records, other paperwork and medication supply must be fully resolved at least 48 hours before admission.
  • The family provides transportation, incontinence products and special dietary or health items.
  • While our rooms are all very tastefully furnished, you may decorate the room as uniquely as you like, to make it resemble the environment your relative enjoyed before joining us. Visit our Photo Gallery or visit live for a TOUR to view sample rooms, some very "personalized."
  • Single room, personalized
  • If a resident has to go temporarily to a hospital, skilled nursing facility or rehabilitation center, family members may arrange to have us hold their room so they can return "home" as soon as possible. Our staff will visit the resident in the other facility and coordinate re-admission with health care providers and family, if they remain appropriate for The Country Home.
  • If a resident needs supplementary services at The Country Home, such as speech therapy, physical therapy, or podiatry, our staff can help coordinate the services with family and service providers.

Short Term Stays or "Respite Care:"

Short term residential care—a rest for care givers and a stimulating social experience, health care and supervision for your loved one! You go on vacation, a business trip, or just take a much needed break from the stress and exhaustion of caregiving. Short stays can also serve as an excellent way to make a gentle transition to permanent residential care. After a few successful short stays, you and your loved one will both feel "at home" in The Country Home, knowing we are here for you when the time is right.

  • $160 day private room, $145 day shared room; based on availability. We maintain a waiting list from time to time. (Fee payable on the first day of service.)
  • Same admission process and requirements as for a permanent resident;
  • Prior to admission, short stay residents must have a two-step TB test, a MRSA test and a flu vaccination, during flu season.

  • Like permanent residents, those coming for a short stay enjoy all the services and activities of a permanent resident
  • Minimum of three, maximum of 30 consecutive days, when available
  • Medical records, other paperwork and medication supply must be fully resolved at least 48 hours before admission. Depending on the interval between short stays, paperwork and pre-tests might need updating.
  • The family provides transportation, incontinence products and special dietary or health items.

The Day Club

One of the most valuable services for dementia/Alzheimer's patients and their exhausted caregivers is a social or medical day program. These day programs can relieve the tremendous stress on caregivers on an occasional or regular basis. In addition to our permanent and short stay residential services, The Country Home accepts members in its Day Club when space is available. Whether the caregiver just needs a full-day break from the demands of caregiving, or has personal, family, business obligations, The Country Home will care for your loved one just as you do. Using our Day Club will acquaint you and your loved one with valuable services outside your home, in case you need a short stay or a transition to residential care. Compare the costs of having an aide stay with your loved one at home for 8-10 hours—and do nothing all day--with our very modest cost to have him or her enjoy the supportive, stimulating, safe environment in The Day Club!

  • Available 8:00 A.M. to 6:00 P.M. seven days a week
  • The Club member enjoys meals and snacks and participates in all the activities and services that regular residents enjoy, with the same supervision and support.
  • $60 a day for an independent senior who needs no ADL assistance
  • $75 a day for a senior who needs ADL assistance and/or medication management (Fee payable on the day of service.)
  • We do not offer a "half day rate" but the Club Member may stay a minimum of four hours and a maximum of ten hours between 8:00 A.M. and 6:00 P.M. Club members usually attend a minimum of five days a month.
  • Sing-a-longs for residents and Day Club guests
  • All admission paperwork and medication supply must be fully resolved at least 48 hours before admission to The Club. Paperwork and physicians' orders will be valid for a period of six months. Any new physicians' orders/medications must be provided 48 hours prior to each visit, if changes occur.
  • Prior to admission, Day Club members must have a two-step TB test, a MRSA test and a flu vaccination, during flu season.
  • The Club member's family provides transportation, incontinence products, a change of clothing and special dietary or health items.
